Procurement of Warehousing Services for the Period December 17-31, 2019
NOTICE IS HEREBY GIVEN that the National Food Authority Abra Provincial Office, thru its Provincial Procurement Committee (PPC), intends to procure Warehousing Services through the Lease of warehouse Building/Space for the Storage of Palay and/or other grains/non-grains commodities, details of which are as follows:

Within Abra 1 500 sq. m. P 75.00/sq m./month or a total of P18, 145.16 P 500.00
NOTE:
1. The floor area for bidding is the aggregate of multiple privately-owned warehouses in the province of Abra. Private warehouse owners with warehouse/s having a minimum floor area of a single warehouse of 500 sq. m may participate in the bidding.
2. The minimum height of the warehouse from floor to eaves shall be 6 meters.
3. Actual area measurement per warehouse and the awarded rental or lease rate/shall be the bases of payment.
4. Duration of warehouse lease is from December 17, 2019 to December 31, 2019.
The total Approved budget for the Contract (inclusive of VAT) for the warehousing services is Pesos: Eighteen Thousand One Hundred Forty-Five (P18, 145.16) to be sourced from 2019 Corporate Budget of National Food Authority. Interested suppliers shall use the attached Request for Quotation Form. The Pre-bidding Conference shall be held on December 16, 2019 at 11:00AM and Submission of Quotation Form in Sealed Envelope to the Provincial procurement Committee, National Food Authority AbraProvincial Office, Bangued, Abra shall be on December 16, 2019 at 11:15AM, together with the certified Photocopies of the following documents:
1. PhilGEPS Certificate of Membership under Platinum Category or Official Receipt as prrof of application/ payment for platinum PhilGEPS membership
2. Mayor-s/Business Permit CY 2019
3. Registration Certificate
4. Tax Clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by BIR
5. Audited financial statements for the year 2018 stamped received by the BIR such as:
a. Balance Sheet
b. Income Statement
c. Independent Auditor-s Certificate that he/she has audited the Financial Statements
d. Cash Flow
6. Business Tax Returns for the 1st and 2nd Quarter of 2019
7. Omnibus Sworn Statement (Annex A)
8. Proof of Ownership
9. Authority of the Signatory (if applicable)
