Description : Surrey Heartlands Integrated Care Board seeks to commission a digital platform solution to support the delivery of a patient-centred model, to deliver multi-disciplinary and collaborative health services. The system should be agile and have the flexibility to respond to evolving technology and operational requirements as these emerge. Surrey Heartlands is seeking a digital platform solution which is clinical pathway agnostic, across an age and care continuum. The platform will provide the interface between patients and clinicians with the ability to enable remote monitoring.Surrey Heartlands ICB are seeking to procure a digital platform to cover an initial 200 Virtual Ward Beds and in addition, to reserve the right to commission further services from the successful provider covering:• wearable remote monitoring devices and associated connectivity to the platform• incorporation of existing patients• additional virtual ward bedsIt is a requirement that the Digital Platform is implemented and operational in each of the four Places within 12 weeks of contract start date of 1 September 2023 and this is estimated to be by 31 December 2023.
