Description : The Presidential Management Staff (PMS), through Administrative Service - Procurement Management Division (AS-PMD), intends to procure Various Vaccines (see attached) with an Approved Budget for the Contract amounting to Five Hundred Thirty Three Thousand Ninety Eight Pesos 30/100 (P533, 098.30) through Section 34 (Small Value Procurement) of the Implementing Rules and Regulations of Republic Act (R.A.) No. 12009.

a. PhilGEPS Registration Number;
b. Mayor-s/Business Permit ;
c. Annual Income/Business Tax Return;
d. Notarized Omnibus Sworn Statement and if applicable, Original Notarized Secretary-s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney .
