GENERAL
• A single-use healthcare plan with coverage for emergency care and hospitalization of cases due to accidents, bacterial and viral infections.
• HMO provider must comply with the requirements under the provisions of Department of Budget and Management (DBM) Budget Circular No. 2024-6, such as but not limited to Insurance Commission registration.
• With at least four (4) accredited hospitals, clinics or diagnostic centers in each of the provinces of Region I (La Union, Pangasinan, Ilocos Sur, and Ilocos Norte) and at least 300 accredited hospitals nationwide
• Coverage period of at least one (1) year
• Principal enrollees must be between 18 and 65 years of age
• Maximum financial assistance for accidental death and disability benefit of at least Fifty Thousand Pesos (P50, 000.00) per year
• Annual mode of payment
EMERGENCY CARE AND HOSPITALIZATION, inclusive of at least the following:
• Room and board
