Tondo Medical Center, through its Bids and Awards Committee (BAC), intends to procure the project Procurement of Sheet-Fed Document Scanner with an Approved Budget for the Contract of Five Hundred Eighty-Five Thousand Pesos (Php 585, 000.00) through Section 34 Small Value Procurement of the Implementing Rules and Regulations of Republic Act No. 12009.
Please submit the following documentary requirements addressed to the BAC Chairperson on or before 9:00 PM of 13 May 2026 at procurement@tmc.doh.gov.ph or bac@tmc.doh.gov.ph, subject to the compliance with the Instructions and Terms and Conditions provided on this Request for Quotation (RFQ):
1. Duly accomplished and signed Technical Specifications using the attached form;
2. Duly accomplished Price Schedule using the attached form;
3. Current Valid Certificate of PhilGEPS Registration / PhilGEPS registration number;
