Procurement of one (1) unit Digital X-Ray Machine

The San Jose City Local Government through the General Fund intends to apply the sum of Two Million Eight Hundred Seventy Five Thousand Pesos ( P2, 875, 000.00 ) being the Approved Budget for the Contract (ABC) to payment under the contract for the Purchase of One (1) Unit Digital X-Ray Machine. Bids received in excess of the ABC shall be automatically rejected at the bid opening.

-c Current and Valid Certificate of Manufacturer-s compliance with ISO 13485

-c Certification to provide user-s Manual in English Language

a. Operations Manual 2 copies

b. Service Manual 2 copies

-c Certification from the legal manufacturer that the supplier has the capability for the corrective and preventive maintenance of the unit.

-c Certification from the supplier that they will provide a backup unit in case of repair of equipment.

-c Certificate of Commitment from the legal manufacturer of the unit that they will honor the warranty in case of change of distributor/supplier

-c Transportation to site, delivery installation and testing expenses on the site (health facility) for the account of the supplier and notification of the supplier to the recipient and respective regional offices prior to the delivery

-c The manufacturers certificate that the brand must be in the local market for at least five (5) years and 10 years in the international market

Instrument Installation

The supplier shall install the unit free of charge

Training Requirement

-c The supplier must provide training, orientation and product demonstration for users and maintenance personnel of the health facility

-c Certificate of training must be noted/signed by the head of Facility

After Sales Service

-c Bidder-s certificate that parts shall be available at the authorized Philippine service center/s for a period of (5) years after the cessation of production of the same model.

-c List of service centers in major cities where equipment is installed and supplies/ reagent outlet with contact landline numbers should be available

-c Preventive maintenance every six (6) months

Warranty Certificate

-c Minimum of three (3) years warranty on parts and three (3) years on service Warranty Certificate upon delivery, inspection and acceptance and after the provision of the required training of the end-user.

The San Jose City Local Government now invites bids for the Purchase of One (1) Unit Digital X-Ray Machine. Delivery of the Goods is required within sixty (60) calendar days upon receipt of Notice to Proceed. Bidders should have completed within the last two (2) years from the date of submission and receipt of bids, a contract similar to the project. The description of an eligible bidder is contained in the Bidding Documents particularly, in Section II, Instruction to Bidders.



Bidding will be conducted through open competitive bidding procedures using a non-discretionary -œpass/fail- criterion as specified in the Implementing Rules and Regulation (IRR) of Republic Act (RA) 9184, otherwise known as the -œGovernment Procurement Reform Act-. Bidding is open to all interested bidders, whether local and foreign subject to the conditions for eligibility provided in the IRR of RA 9184.



Interested bidders may obtain further information from the City of San Jose and inspect the Bidding Documents at the address given below during Mondays thru Fridays, 8:00 AM-12:00 NN and 1:00 P.M. - 5:00 P.M.



A complete set of Bidding Documents may be purchased by Interested Bidders on July 18, 2018 up to August 7, 2018 from the address below and upon payment of non-refundable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, in the amount of five thousand pesos (P 5, 000.00).



It may also be downloaded free of charge from the website of the Philippine Government Electronic Procurement System (PhilGeps) and the website of the Procuring Entity provided that Bidders shall pay the non-refundable fee for the Bidding Documents .



San Jose City will hold the following schedule of Public Bidding Activities at 10:00A.M. at the City General Services Office of this city;

1) Pre-Bid - July 24, 2018

2) Deadline for submission, opening of bid- August 7, 2018

3) Post Qualification & Evaluation of Bid - August 8 - 13, 2018

4) Awarding of Contracts - August 14, 2018

The City of San Jose reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder/s.
