Description : TERMS AND CONDITIONS
1. All line item/s under this project shall be procured as ONE LOT and interested suppliers are required to submit two (2) copies each of their valid and current (a) 2026 Mayor-s/Business Permit, (b) PhilGEPS Registration Number/Certificate and (c) ORIGINAL copies of the attached Price Quotation Form (Annex “A”).
2. Canvass Period starts on July 17 up to July 23, 2026. Submission of quotation and eligibility is on or before 4:30 pm of July 23, 2026 at Office of the Bids and Awards Committee, Administrative Building, DOH-DTRC Complex, Bonuan Binloc, Dagupan City, Pangasinan 2400. Open submission may be submitted, manually or electronically through [email protected]
3. Quotations shall be inclusive of applicable taxes and other indirect costs, those exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.
