Description : TERMS AND CONDITIONS
1. All line item/s under this project shall be procured as ONE LOT and interested suppliers are required to submit two (2) copies each of their valid and current (a) 2026 Mayor-s/Business Permit (b) PhilGEPS Registration Number/Certificate, (c) Omnibus Sworn Statement, (d) FDA License to Operate as Drug Retailer/Drugstore and (e) Original copies of the attached Price Quotation Form (Annex “A”). Submission of Certificate of Product Registration is during delivery and/or prior to payment of Contract.
2. Canvass Period starts on April 23 up to April 29, 2026. Submission of quotation and eligibility is on or before 4:30 pm of April 29, 2026 at Office of the Bids and Awards Committee, Administrative Building, DOH-DTRC Complex, Bonuan Binloc, Dagupan City, Pangasinan 2400. Open submission may be submitted, manually or electronically through [email protected]
3. Quotations shall be inclusive of applicable taxes and other indirect costs, those exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.
