Description : TERMS AND CONDITIONS
1. All line item/s under this project shall be procured as ONE LOT and interested suppliers are required to submit two (2) copies each of their valid and current (a) 2026 Mayor-s/Business Permit, (b) PhilGEPS Registration Number/Certificate, (c) Omnibus Sworn Statement, (d) 2025 Income/Business Tax Return (Form 1701/1702 - All pages) (e) FDA License to Operate as Drug Retailer/Drugstore and (f) Original copies of the attached Price Quotation Form (Annex “A”). Submission of Certificate of Product Registration is during delivery and/or prior to payment of Contract.
2. Canvass Period starts on July 17 up to July 23, 2026. Submission of quotation and eligibility is on or before 4:30 pm of July 23, 2026 at Office of the Bids and Awards Committee, Administrative Building, DOH-DTRC Complex, Bonuan Binloc, Dagupan City, Pangasinan 2400. 3. Quotations shall be inclusive of applicable taxes and other indirect costs, those exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.br...
