Description : TERMS AND CONDITIONS
1. All line item/s under this project shall be procured as ONE LOT and the identified supplier is required to submit two (2) copies each of their valid and current (a) 2026 Mayor-s/Business Permit, (b) PhilGEPS Registration Number/Certificate, (c) Omnibus Sworn Statement, (d) 2025 Income/Business Tax Return (Form 1701/1702 - All Pages) and (e) ORIGINAL copies of the attached Price Quotation Form (Annex “A”).
2. Canvass Period starts on July 18 up to July 24, 2026. Submission of quotation and eligibility is on or before 4:30 pm of July 24, 2026 at Office of the Bids and Awards Committee, Administrative Building, DOH-DTRC Complex, Bonuan Binloc, Dagupan City, Pangasinan 2400. Open submission may be submitted, manually or electronically through [email protected]
3. Quotations shall be inclusive of applicable taxes and other indirect costs, those exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.
4. BAC reserves the right to reject any or all Quotations, to waive any minor defects therein, to annul the procurement process, to reject all Quotations at any time ...
