1. The Municipal Government of Benito Soliven, Isabela through its Bids and Awards Committee, will undertake Small Value Procurement in accordance with section 34 of the Implementing Rules and Regulations of Republic Act No. 12009 or the New Government Procurement Act.
2. The Municipal Government of Benito Soliven, Isabela hereinafter referred to as End- User requests interested applicants to submit quotation.

Interested suppliers are required to submit their valid and current Mayor's Business Permit, PhilGeps registration Number, Certificate of registration (whichever is applicable0, A Sworn affidavit (Appendix "I"), Statement of All its Completed Contracts and/or list indicating work experience of their members that reflect capacity to deliver the goods, Photocopy or scanned copy of bankbook with complete bank account information and proof of cash receipts journal, cash disbursement journal, general journal and ledger in a sealed envelope marked: (Envelope 1-Name of the Project-Eligibility Requirements).
