1.1 Valid and current Mayor-s/Business permit issued by the city or municipality where
the principal place of business location, or the equivalent document for Exclusive Economic Zones
1.2 Valid certificate or proof of PhilGEPS Registration (Platinum/Red Membership, as applicable under IRR)
1.3 Duly notarized Omnibus Sworn Statement (Form No 2).
Note: A valid PhilGEPS Platinum Registration Certificate may be accepted in lieu of the required eligibility documents, subject to verification and the submission of additional supporting documents, if deemed necessary by the Procuring Entity in accordance with the IRR of RA 12009.
