(i) Registration certificate from the Securities and Exchange Commission (SEC) for corporations, Department of Trade and Industry (DTI) for sole prorietorship, or Cooperative Development Authority (CDA) for cooperatives, whichever is applicable;
(ii) Mayor`s/Business Permit issued by the city of the municipality where the principal place of business of the prospective bidder is located, or the equivalent document for Exclusive Economic Zone or Areas;
(iii). Professional License/Curriculum Vitae;
(iv) PhilGEPS Registration Number;
(v) Latest income and business tax returns:
(iv) Omnibus Sworn Statement (Reference 2016 Revised IRR of RA9184)
2.)Prices are inclusive of taxes, delivery and other incidental costs.
3.) Delivery schedule must be on September 29, 2017.
4.) The deadline for submission of quotation shall be on Sept 29, 2017 11:00AM. Opening of the Bids will be on Sept 29, 2017 2:00PM. The bidders shall send their duly accomplished quotation form together with the eligibility requirements.
