TERMS OF REFERENCE

I. PROJECT AND LOCATION

PROJECT: SUPPLY AND DELIVERY OF CELLCARDS FOR TWELVE (12) MONTHS CONSUMPTION

LOCATION: PHILIPPINE COCONUT AUTHORITY DILIMAN, QUEZON CITY

II. SCOPE OF WORK

The work to be performed shall be:

1. Supply and delivery of the following Cellcards:

• 140 pcs Globe Prepaid card, 500

• 34 pcs Globe Prepaid card, 300

• 378 pcs Globe Prepaid card, 100

• 90 pcs Smart Prepaid card, 500

• 10 pcs Smart Prepaid card, 300

• 176 pcs Smart Prepaid card, 100

• 28 pcs TM Prepaid card, 500

• 24 pcs TM Prepaid card, 100

2. Delivery and payment of cellcard shall be on a monthly basis

3. ALL REGULAR LOADS ONLY. (Note: Expiration should atleast one (1) year from the date of delivery.

TOTAL ABC: ₱ 200, 000.00
