Description : TERMS AND CONDITIONS
1. All line item/s under this project shall be procured as ONE LOT and interested suppliers are required to submit two (2) copies each of their valid and current (a) 2026 Mayor-s/Business Permit, (b) PhilGEPS Registration Number/Certificate, (c) Omnibus Sworn Statement, (d) Income / Business Tax Return 2025 (BIR Form 1701 /1702) and (e) ORIGINAL copies of the attached Price Quotation Form (Annex “A”).
2. Canvass Period starts on July 1 up to July 7, 2026. Submission of quotation and eligibility is on or before 4:30 pm of July 7, 2026 at Office of the Bids and Awards Committee, Administrative Building, DOH-DTRC Complex, Bonuan Binloc, Dagupan City, Pangasinan 2400. 3. Quotations shall be inclusive of applicable taxes and other indirect costs, those exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.
