Procurement of Accountable Forms for the Use of the Provincial Treasurer-s Office
SOURCE OF FUND: GENERAL FUND [1091-200-5-02-03-020]
1. The Provincial Government of Davao Oriental through the Bids and Awards Committee (BAC) intends to apply the sum of NINE HUNDRED NINETY FOUR THOUSAND FIVE HUNDRED PESOS (P994, 500.00) being the Approved Budget For Contract (ABC) to payments under the contract for the Procurement of Accountable Forms for the Use of the Provincial Treasurer-s Office. Bids received in excess of the ABC shall be automatically rejected at Bid opening.
2. The Provincial Government of Davao Oriental now invites bids for the Procurement of Accountable Forms for the Use of the Provincial Treasurer-s Office. Delivery of the goods is required within 7 days. Bidders should have completed, within 2 years from the date of submission and receipt of bids, a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly in Section II- Instruction to Bidders.
3. Bidding will be conducted through open competitive bidding procedures using non-discretionary pass/fail criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act 9184 (R.A. 9184), otherwise known as the -œGovernment Procurement Reform Act-.
4. Interested Bidders may obtain further information from BAC Secretariat and inspect the bidding documents at the address given below during office hours.
5. A complete set of Bidding Documents may be purchased by interested bidders on April 12-22, 2019 from the address below and upon payment of a non-refundable fee for the Bidding Documents in the amount of ONE THOUSAND PESOS (Php1, 000.00).
6. Bids must be delivered to the address below on or before April 22, 2019 - 2:00 PM. All bids must be accompanied by a Bid Security in any of the acceptable forms and in the amount stated in ITB Clause 18.
Form of Bid Security - Cash or cashier-s/manager-s check issued by a Universal or Commercial Bank (2% of ABC)
Minimum Amount - Php 19, 890.40
Form of Bid Security - Bank draft/guarantee or irrevocable letter of credit issued by a Universal or Commercial Bank. Provided, however, that it shall be confirmed or authenticated by a Universal or Commercial Bank, if issued by a foreign bank ( 2% of ABC)
Minimum Amount - Php 19, 890.40
Form of Bid Security - Surety Bond callable upon demand issued by a surety of insurance company duly certified by the Insurance Commission as authorized to issue such security (5% of ABC)

7. Bid opening shall be on April 22, 2019 - 2:00 PM at the BAC Conference Room, Capitol Hill, City of Mati. Bids will be opened in the presence of the Bidders or their representatives who choose to attend. Late bids shall not be accepted.
8. The Provincial Government of Davao Oriental reserves the right to accept or reject any Bid, to annul the bidding process, and to reject all Bids at any time prior to contract award, without thereby incurring any liability to the affected Bidder or Bidders.
