REQUEST FOR QUOTATION

To All Eligible Supplier:

Please deliver or send your price quotation in hard copy of the attached Quotation Form in a sealed envelope with the name of this procurement, -œPrinting of Office Forms (3rd Qtr 2019)- on the outside, signed by an authorized representative of the supplier, to the address listed below. The offered price shall not exceed the Approved Budget Contract (ABC) of Five Hundred Thousand Pesos (a‚+/-500, 000.00) and shall include all costs of delivering the goods. Offers received later than 9 A.M. August 27, 2019 will not be considered. Submit to BFP Region VIII, Bids and Awards Committee, J. Romualdez and P. Paterno Sts., Tacloban City. Kindly submit the following documents in Five (5) authenticated sets, to wit:

1. Quotation Form with the Supplier-s Bid;

2. Registration Certificate from SEC for Corporations, Department of Trade and Industry (DTI) for sole proprietorship, or CDA for cooperatives, or any proof of such registration;

3. Valid and current Mayor-s permit issued by the city or municipality where the principal place of prospective bidder is located;

4. PhilGEPS registration certificate/number;

5. Income/Business Tax Return; and

6. Omnibus Sworn Statement.

Delivery period is within Two (2) Calendar Days after dual signature of Purchase Order and receipt of Notice to Proceed. All quotations should be valid for sixty days from RFQ submission deadline. Prices shall be in Philippine Peso (Php).
