REQUEST FOR QUOTATION
1. The Bangko Sentral ng Pilipinas requests quotations for the hereunder requirement:

Item 1: One (1) lot Printing and Blister Packaging of Mariano Ponce Commemorative Coins (5, 000 sets)
Item 2: One (1) lot and Blister Packaging Pio Valenzuela Commemorative Coins (5, 000 sets)
Approved Budget for the Contract (ABC) : Item 1: PhP325, 000.00, inclusive of all applicable taxes
Item 2: PhP325, 000.00, inclusive of all applicable taxes
(Quotations/Proposals exceeding the ABC per item shall be disqualified)
Delivery Period : Within thirty (30) calendar days from receipt of coins. Pick-up of coins by the awardee shall be within seven (7) calendar days reckoned on the date stipulated in the Notice to Proceed to be issued by the Mint and Refinery Operations Department (MROD)

2. All particulars and activities relative to Eligibility of Suppliers, Evaluation of Quotations/Proposals, Post-qualification and Award of Contract shall be governed by 2016 Revised Implementing Rules and Regulations of Republic Act No. 9184 (2016 Revised IRR).

3. You may send your quotation thru fax or email as specified above on or before the deadline of submission of Financial Proposals (use BSP Prescribed Form). Prospective suppliers shall have the option of submitting quotations on any or all items and evaluation and contract award will be undertaken on a per item/lot basis. Late submission of Financial Proposals will not be accepted.
4. The Supplier with the Single or Lowest Calculated Quotation shall submit, within five (5) calendar days upon receipt of notice from this Office, the following documents:
a. Legal Documents:
a.1. PhilGEPS Registration Certificate;
a.2. Y2019 Mayor-s/Business Permit (disregard if you have already submitted/updated the same) or in case of recently expired permit, submit copy thereof together with the Official Receipt for its renewal; and
NOTES:
i. For Joint Venture: Each partner shall submit this document;
ii. The renewed Mayor-s/Business Permit shall be submitted as a post-qualification requirement; and
iii. For Individuals engaged under Sec. 53.6, 53.7 and 53.9 of the 2016 Revised IRR, only the BIR Certificate of Registration shall be submitted in lieu of DTI Registration and Mayor-s Permit.
a.3. Omnibus Sworn Statement (use BSP prescribed form).
b. Financial Document: Y209 Income Tax Return (ITR)/Business Tax Return for the quarter ending 30 June 2019 duly filed thru Electronic Filing and Payment System (eFPS) of the BIR, as provided for under E.O. 398 or RR 3-2005, together with the Filing and Payment References (disregard if you have already submitted/updated the same);
c. Technical Documents:
b.1. Valid JV Agreement (JVA), if applicable;
b.2. BSP Terms and Conditions duly signed by the supplier-s authorized representative on the CONFORME page (1 page);
