Preparation and delivery of layettes for children born in provincial medical entities with option rights 1. The subject of the order is the production and delivery of layettes for children born in provincial medical entities in 2026 in the amount of 1, 149 sets (basic order) with option rights (1, 149 sets). 2. The Department of Family plans to implement the task of providing a symbolic gift for children born in provincial health care entities in 2026. These will be layettes donated in three provincial medical entities (Ludwik Rydygier Specialist Hospital in Kraków sp. z o. o., St. Luke's Provincial Hospital in Tarnów and Jędrzej Śniadecki Specialist Hospital in Nowy Sącz) to parents of children after birth. 3. The layette will consist of a pillow, a quilt, a mascot and a cotton bib with the WM logo, packed in a window box tied with a ribbon. 4. A detailed description of the subject of the order is attached as Annex No. 1D to the SWZ at the same time, Annex No. 1 to the contract. 5. The terms and conditions of the contract are included in the draft provisions of the public procurement contract, which will be included in the content of this contract - the contract template and constitute Annex No. 1C to the SWZ (hereinafter referred to as are hereinafter referred to as a model contract). 6. The Ordering Party provides for the possibility of exercising the option right under this order.
