REQUEST FOR QUOTATION

The Local Government Unit of Siaton, Siaton Negros Oriental, through the Bids and Awards Committee (BAC) will undertake a Small Value Procurement for the -œPurchase of 2, 000 liters of Diesel Fuel- to be used by the PNP Siaton, in accordance with Sec. 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.

Brief Description : Purchase of 2, 000 liters of Diesel Fuel to be used by the PNP Siaton

Location : Siaton, Negros Oriental

Delivery Term : 60 CD

Interested suppliers are required to submit their valid and current Mayor-s Permit and PhilGEPS Registration Number upon submission of quotation. PhlGEPS Platinum Membership may be submitted in lieu of the Mayor-s/Business Permit and PhilGEPS Registration Number.

Award of Contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form.

Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.

Submission of quotation and eligibility documents is on or before 10:00 A.M. of November 4, 2019 at the Bids and Awards Committee Office at the 2nd Floor, Mayors Office, Siaton Municipal Hall.
