SHA Health Information Management is requesting an RFP for vendor supplied patient ID labels that are used to label clinical forms for patients that are registered to an acute care facility in Regina, Saskatoon, and surrounding rural areas. Saskatoon acute care will use approximately 400, 000 sheets of labels per year. Regina acute care will use approximately 330, 000 sheets of labels per year. Regina acute care will use approximately 1, 100 three (3) inch rolls of labels per year. Regina acute care will use approximately 200 one (1) inch rolls of labels per year.

Patient ID labels will be used in the following locations: Royal University Hospital – Saskatoon Saskatoon City Hospital – Saskatoon St. Paul’s Hospital – Saskatoon Jim Pattison Children’s Hospital – Saskatoon Regina General Hospital – Regina Pasqua Hospital – Regina Wascana Rehab Centre – Regina Humboldt Hospital – Humboldt Lanigan Hospital – Lanigan Rosthern Hospital – Rosthern Wadena Hospital – Wadena Watrous Hospital – Watrous Wynyard Hospital – Wynyard All Nations Healing Hospital – Fort Qu’Appelle Balcarres Integrated Care Centre – Balcarres Broadview Hospital – Broadview Indian Head Hospital – Indian Head Southeast Integrated Care Centre – Moosomin Wolseley Memorial Hospital - Wolseley SPECIFICATIONS/TECHNICAL REQUIREMENTS Saskatoon and area acute care requires an 8.5 X 11 sheet of labels with 21 labels per sheet.
