This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6, as supplemented with additional information included in this notice.
Synopsis: the department of veterans affairs, visn 6 network contracting office, on the behalf of the kernersville va health care center has a need for kleinsasser panendoscopy instruments. This is a brand name only requirement to match existing sets of panendoscopy instruments. This requirement is for new equipment only; no remanufactured or "gray market" items. All items must be covered by the manufacturer s warranty. All offerors must be authorized distributors of the equipment.
Solicitation: see attached solicitation 36c24618q9261. This solicitation has been set-aside for small businesses, including sdvosbs and vosbs. Offeror must be registered on sam website, and vetbiz (vip) if they are a sdvosb or vosb.
Quotations shall include proposed unit prices, proposed extended prices, any prompt payment discount terms, any shipping charges, company name, and point of contact, address, phone number, tax identification number, and duns number.
