In case of recently expired Mayor-s Permit, it shall be accepted together with the official receipt as proof that the supplier has applied for its renewal.
• Valid PhilGEPS Registration Number/Certificate
• Income Tax Returns for the last two (2) quarters filed through EFPS
• Original and Notarized Omnibus Sworn Statement
