Please see the attached Purchase Request for Approved Budget for the Contract (ABC) and Terms of Reference.
Suppliers will need to submit with the proposal/quotation the following documentary requirements:
1. Supplier's 2019 Mayor's/Business Permit; and
2 . Platinum PhilGeps Certificate of Registration or Red PhilGeps Certificate of Registration with SEC Certificate
3. List of Previous Clients (Private and Public Sector) which indicates the following details: (1. Name of the client, 2. Name/title of the training, and 3. Date Conducted)
4. Certified True Copy of the Certificate from the CSC as accredited Learning and Development Institution
5. Curriculum Vitae (CV) of the Instructor/Designated Resource Speaker
Failure to submit the above-mentioned documents with the proposal/quotation before the deadline of 03 June 2019, 2:00pm renders the submission, NON-COMPLIANT.
Pursuant to DBM Circular No. 2013-16 dated 23 Dec. 2013 (please see attached), the GCG has adopted the Direct Payment Scheme (DPS) via bank debit system through issuance of List of Due and Demandable Accounts Payable Authority to Debit Account (LDDAP-ADA) in settlement of payables due to creditors/payees.
