Description : NWL CCG, requires a locally based support service in Harrow for Somali refugees, asylum seekers and EU migrants, adults with mental health needs, chronic diseases (Diabetes, Heart condition, kidney disease, Alzheimer, Stroke), SENs, learning difficulties, elderly, physically and/or sensory impaired, adults requiring palliative care services, and adults with drug and alcohol problems.
