Description : NWL CCG, requires a locally based support service in Harrow for Somali refugees, asylum seekers and EU migrants, adults with mental health needs, chronic diseases (Diabetes, Heart condition, kidney disease, Alzheimer, Stroke), SENs, learning difficulties, elderly, physically and/or sensory impaired, adults requiring palliative care services, and adults with drug and alcohol problems. Key services required include:-Language skills and cultural awareness to serve the local Somali community.-Improve case management through outreach and sign posts.-Prevent illnesses and premature deaths amongst BME groups.-Provision of health information materials and Harrow Health Guidance in local language.-Workshops (smoke cessations, weight loss, walking and nutrition groups).-Sessions on adolescence changes and substance misuse/alcohol misuse.-Assist KAT users to modify their behaviours with changes in legislation.-Promote the use of walk-in clinics.
