NOTICE OF INTENT TO ISSUE AN ORDER. This is not a request for competitive proposals/quotes. The Centers for Disease Control and Prevention (CDC) hereby announces its intent to issue a sole source order to SPT Labtech Limited, at Unit H4 Melbourn Science Par, Cambridge Road, Melbourn, Herts SG8 6HB, United Kingdom for Maintenance and service agreement for the Arktic freezer..

INTRODUCTION
The Division of Viral Hepatitis, Laboratory branch owns an automated SPT Arktic freezer that is required for storage of clinical isolates associated with outbreak investigations and molecular surveillance. This instrument is indispensable for our laboratory standard operating procedure. As a result, the instrument must be serviced regularly by the vendor. SPT Labs is the solely manufacturer of all Arktic automated freezers.
PURPOSE STATEMENT
The objective of this request is to procure the corresponding SPT service agreement for the Arktic unit own by the laboratory branch. The service agreement should include the following items: Arktic freezer.
SCOPE OF WORK
The service agreement will cover the Arktic instrument. The corresponding service agreement will cover all repairs, replacements, and labor incur by instrument failures.
TASKS
The instruments will be repaired upon failure and the response should not exceed five business days.
Telephone and remote access support.
Software updates (hardware upgrades associated with software upgrades are not included).
Remote application support.
Annual preventative maintenance visit (only applicable to contract terms over 6 months). includes travel, expenses and labor and includes replacement parts.
On-site repair and refurbishment. Includes travel, expenses and labor and replacement parts.
