I.TECHNICAL PROPOSAL
LIST OF ELIGIBILITY REQUIREMENTS
A. Legal Documents
1. Registration certificate from SEC, Department of Trade and Industry (DTI) for sole propriertorship, or CDA for cooperatives.
2. Mayor's/ Business Permit (Current year) issued by the city or municipality where the place of business prospective bidder is located.
3. Certificate of Registration from Bureau of Internal Revenue (BIR)
- where TIN number and address is indicated;
4. Tax Clearance Certificate from Bureau of Internal Revenue (BIR);
5. Certificate of Philippine Government Electronic Procurement System (PhilGEPS) Registration;
6. Valid joint venture agreement (in case of joint venture).
B. Technical Documents
1. Statement of the prospective bidder of all its ongoing/completed Government and private contracts, including contract
awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to bid.
2. Statement of the bidder's Single Largest Completed Contract (SLCC) similar to the contract to be bid, except under conditions
provided for in Section 23.4.1.3 and 23.4.2.4 of this IRR within the relevant period as proviced in the Bidding Documents
in the case of Goods;
3. Omnibus Sworn Statement (Affidavit)
Shall contain the following:
3-a. The signatory is the duly authorized representative of the prospective bidder, and grandted full power and authority
to do, executer and perform any and all acts necessary to participate, submit bid, and to sign and execute the ensuing
contract accompanied by the duly notarized Special Power of Attorney, Board/Partnership Resolution, or Secretary's
Certificate, whichever is applicable;
3-b. It is not blacklisted or barred from bidding by the GoP or any of its agencies, offices, corporations, or LGUs, including
foreign government/foreign international financing institution whose blacklisting rules have been recognized by the GPPB;
3-c. Each of the documents submitted in satisfaction of the bidding requirements is an authenticated copy of the original,
complete, and all statements and information proviced therein are true and correct;
3-d. It is authorizing the HOPE or his duly authorized representative/s to verify all the documents submitted;
3-e. It complies with the disclosure provision under Section 47, of the Act and this IRR;
3-f. It complies with exsting labor laws and standards;
3-g. It complies with the responsibilities of a prospective or eligible bidder provided PBD's;
3-h. It did not give or pay, directly or indirectly, any commission, amount, fee, or any form of consideration, pecuniary or
otherwise, to any pesrson or officials, personnel or representative of the government in relation to any procurement
project and activity;
4. DA-BPI- Accreditation Certificate (for Supplier of Fruit Seedlings and other Planting materials) (N/A);
5. License- from Fertilizer and Pesticide Authority (FPA) (In case of supplier of Fertilizer and Pesticide);
5.A. Certificate of registration from Bureau of Agriculture and Fisheries Standards (BAFS);
6. Certificate of Registration as Livestock Dealer (for Livestock Dealer); (N/A)
6-a. Certificate of Livestock Handler.(N/A)
7. Production/ Delivery Schedule (where it is applicable);
8. After Sales Services (where it is applicable);
9. Warranty Certificate (where it is applicable);
10. Brochure (where it is applicable);
11. Bid Security (to be attached during the Submission/Opening of Bids); in the prescribed form; amount and validity period;

to total amount of security

C. Financial Documents
1. Individual/ Corporation Annual Income Tax Return for the last two (2) years.
2. Audited Financial Statement, showing among others, the prospective bidder's total asset and liabilities, stamped "received" by the
BIR or its duly accredited and authorized institutions, for the preceding calendar year which should not be earlier than two (2) years
from the date of bid submission;
3. Statement of bidder's computation of Net Financial Contracting Capacity (NFCC). However, in the case of procurement of Goods,
a bidder may submit a commitment Line of Credit from a Universal or Commercial Bank, in lieu of its NFCC computation.
NOTE: Bidders may apply for pre-qualification wherein a Certificate of Eligibility shall be issued by this Office. The Authenticated
Photocopy of said Certificate of Eligibility will be submitted in lieu of the Eligibility Requirements.
II. FINANCIAL PROPOSAL
1. Financial Bid in prescribed form.
SEALING AND MARKING OF BIDS
1. The Bidder shall enclose the original documents of the Techinal Proposal (Legal, techical & financial documents) in one sealed envelope
marked as "ORIGINAL- TECHNICAL PROPOSAL"; whereas, the original documents of the Financial Proposal shall be enclosed in another
sealed envelope marked as "ORIGINAL- FINANCIAL PROPOSAL".
2. There shall be two (2) copies of the Technical Proposal and Financial proposal, similarly sealed and duly marked as "COPY NO. 1-TECHINICAL PROPOSAL" and "COPY NO. 1 FINANCIAL PROPOSAL" respectively and the outer envelopes as "COPY NO. 1" and "COPY NO. 2-TECHINICAL PROPOSAL" and "COPY NO. 2 FINANCIAL PROPOSAL" respectively and the outer envelopes as "COPY NO. 2"
3. The legal documents shall be authenticated.
4. These envelopes (containing the original and the copies shall then be enclosed in one single envelope with the following information
indicated therein:
a) Name of the Contract to be Bid;
b) Name and address of the Bidder;
c) Addressed to the PROCURING ENTITY in accordance with ITB
d) Specific identification of this bidding process indicated in the BIDS; and
e) Bear a warning "DO NOT OPEN BEFORE-" the time and date for the opening of Bids, in accordance
with ITB
5. If the envelopes are not sealed and marked as required, the PROCURING ENTITY will assume no responsibility for
the misplacement or premature opening of the Bid.
6. Proper tabbing or a Table of contents with paga number should be provided.
7. Deadline for Submission of Bids
Bids must be received by the BAC of the PROCURING ENTITY at the address and on or before the date and time indicated in the BDS
8. Late Bids
Any Bid submitted after the deadline for submission and receipt of Bids prescribed by the PROCURING ENTITY,
pursuant to ITB, Shall be declared "Late" and shall not be accepted.
FAILURE TO FOLLOW AND SUBMIT ANY OF THE REQUIREMENTS ENUMERATED
SHALL RENDER THE BIDDER INELIGIBLE TO BID/DISQUALIFICATION
If already pre-qualified and a Certificate of Eligibility has been issued by this office, the same must be submitted by the Bidder
(Authenticated Xeroxed copy).
Bidding will be conducted through open competitive bidding procedures as specified in the IRRof the R.A. 9184.
Only Bids from eligible biders will be opened and will be awarded to the lowest and responsive bidder/s that will be determined as
such after comprehensive evaluation/post qualification conducted by the Technical Working Group (TWG).

Bidding documents (prescribed bid Form) will be issued at the office of the Secretariat Bids & Awards Committee, DA-RSO IX, Zamboanga City
during office hours and upon payment of non-refundable amount of One Thousand Four Hundred Pesos (P 1, 400.00) Only.
The DA-RFO IX, reserves the right to reject any or all bid proposals, to waive any required formality or defect therein and may accept the bid/s which is deemed most advantageous to the government, without incurring any liability to the affected bidder or bidders.
