The subject of the order is the implementation of a representative population survey among the residents of the Czech Republic who have sustained the illness of COVID 19. ethene includes: CAWI, CAPI or PAPI questionnaire among 1, 000 persons with full rights to R and current migrants to country R who have overstayed sick with COVID 19, the target population consists of persons between the ages of 18 and 65, who are eligible for R and are currently going to the R country, who have been ill with COVID 19., the file will be displayed according to the specified criteria (see Plot . 2), the absolute number of persons will reach a minimum of 1, 000 persons, Cooperation is expected when finalizing the content of the questionnaire, technical finalization of the questionnaire. The length of the questionnaire/interview is about 10 minutes. The questionnaire form will be provided to the selected supplier by the contracting authority. The precise wording of the question should be in writing with the selected supplier. Inputs: 1. Use a data file in SPSS format with descriptions of variables and categories (labels), (see Plot 2). 2. Technical information about the collection process, quality control and the achieved quantity. Requested entries must be received no later than April 20, 2026.
