3. Description, size and scope of the subject of the order:

1) The subject of the order is the lease of a surgical robot with equipment and the supply of consumablesfacilities adapted to perform procedures

in the field of urology and general and colorectal surgery.

2) The device constituting the subject of the lease meets the requirements specified in the law, in particular in the Act of April 7, 2022 on medical devices and the Regulation on Medical DevicesMinister of Health of January 25, 2022 and of July 28, 2023 amending the regulation on guaranteed benefits in the field of hospital treatment and may be used in surgical treatment using a robotic system.

3) The subject of the order must ensure the performance of 200robotic procedures during the duration of the Agreement (Urology - 140 procedures; General and colorectal surgery - 60 procedures), however, the Ordering Party reserves the right to complete the full scope of the order in a period shorter than 12 months.
