1. The subject of the order is the lease of the ANGIO OCT optical coherence tomograph with equipment for the 2nd Department of Ophthalmology.2. Whenever the phrase: a) equipment is used in the following part of the SWZ - this term should be understood as a leased optical tomograph with equipment, the description and technical requirements of which are included in Annex No. 2 (FPW).3. The description of the subject of the order is included in Annex No. 2 to the SWZ, hereinafter also referred to as FPW), which should absolutely be submitted together with the offer form (Appendix 1 to the SWZ) (hereinafter also referred to as FO).4. The Ordering Party requires that the offered equipment constitutes a medical product within the meaning of the Act of April 7, 2022 on medical devices (Journal of Laws 2024.1620, as amended, hereinafter referred to as: UoWM), placed on the market and used in the territory of the Republic of Poland in accordance with the applicable regulations. 5. The conditions for the execution of the order are specified in the draft provisions of the contract constituting Chapter II of SWZ6. The Ordering Party does not allow the submission of variant offers.7. An order consists of 1 task.8. The Ordering Party requires that the leased equipment be maintained in full working order the duration of the contract, i.e. for 36 months.9.
