The government requires the purchase of various medical equipment repair parts manufactured by pcb piezotronics, inc. Larson davis division. Due to army regulation and food and drug administration (fda) regulations, these items must be purchased from the original equipment manufacturer (oem) or from an authorized reseller. A redacted sole source justification has been attached to this rfq which supports the rationale for accomplishing this requirement on a brand name only basis. Usamraa on behalf of u.s. Army medical materiel agency (usamma) requests responses from qualified sources capable to provide equipment that is brand name to piezotronics, inc. Larson davis division. The following action is to procure brand name piezotronics, inc. Larson davis division medical equipment without competition as prescribed in far 13.106-1(b)(1)(i). This announcement constitutes the solicitation by usamraa, fort detrick, md, as a brand name request for quote (rfq) and offers must be submitted in writing (oral offers will not be accepted). The solicitation document and incorporated provisions and clauses are those in effect through federal acquisition circular (fac) 2005-89; 15 august 2016.



The brand name part needed at usamma depot located in tobyhanna, pa, which replaces medical equipment used by deployed forces is part number sys008 "complete audiometer calibration system" manufactured by piezotronics, inc. Larson davis division.
