1.The City Health Office - Public Information Section intends to procure thru Negotiated Procurement (after Two Failed Biddings) pursuant to Section 53.1 of the 2016 Revised Implementing Rules & Regulations of RA 9184, the project: Laboratory Supplies with an Approved Budget for the Contract (ABC) of Two Hundred Ninety-Six Thousand Nine Hundred Fifty Pesos (P296, 950.00)
2.The City Health Office - Public Information Section, through its Bids and Awards Committee, now invites technically, legally, and financially capable bidders for the said project.
3.Interested bidders shall submit the following documents as well as the Price Quotation at the Office of the City General Services:

c. ITR
d. Omnibus Sworn Statement
