TERMS OF REFERENCE FOR PROCUREMENT OF BLOOD BAGS FOR BLOOD COLLECTION



THIS IS LOT BID



The supplier awarded to supply the blood bags shall adhere to the TERMS OF REFERENCE stated below:

1. Provide for FREE the following:

1a. Blood bags (single, double, triple, and quadruple) and service assistance for product validation/verification, when indicated

1b. Use of TWO (2) complementary heavy duty electronic Tube Sealing Devices (TSD) for in-house and mobile blood donation

- Preferably brand new, but equipment less than 3 years old is acceptable

1c. Use of ONE (1) complementary Blood Component Separator (BCS) for leukoreduction

- Preferably brand new, but equipment less than 3 years old is acceptable

2. Provide for FREE the regular preventive maintenance of TSD and BCS, replacement of parts or units duly coordinated with concerned laboratory staff and EFM personnel.

3. Provide for FREE the training and re-training of laboratory staff to make them competent to operate the TSD and BCS

4. The MOA shall cover a period of 6 months with added stipulation for extended use of TSD and BCS for free in case the blood bags are not be consumed within 6 months.

5. Ensure compliance with scheduled preventive maintenance of TSD and BCS

6. Ensure prompt response by designate engineer/s within 24 hours for correction of equipment failure or malfunction.

7. Replenish consumed bags incidental to inadequate sealing due to equipment failure, malfunction or trials during training/re-training.

8. Assures replacement of unused blood bags (delivered within 6 months of expiration date) before expiration.

9. Products carried are covered by current Certificate of Product Registration or, in the absence of the former, proof of renewal or updated Certificate of Exemption issued by Philippine FDA

10. Manufacturing of reagents and equipment are covered by relevant ISO certification or quality certification of similar nature

11. Supplier/distributor is preferably, but not necessarily, ISO 9001:2015 certified.
