I. AIRFARE :



Six (6) Business Class Tickets

Six (6) Economy Class Tickets



II. MINIMUM REQUIREMENTS FOR SUPPLIERS :



-c International airline tickets

-c Booking class (show in quotation): C (for Business Class) / Y (for Economy Class)

-c Ticket is re-bookable and refundable

-c Ticket validity - one (1) year

-c Regular fare not promo fare

-c Show breakdown of quotation

-c Include travel insurance for DOT employees only

-c Include departure tax and fuel surcharges

-c Must be willing to provide tickets on a send bill arrangement

-c Offices located in Metro Manila only

-c Must be DOT accredited

VI. LEGAL / TECHNICAL REQUIREMENTS TO BE SUBMITTED BY THE BIDDER:



a) Registration Certificate from SEC or DTI or CDA;

b) Current Mayor-s Permit;

c) Tax Clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by the BIR;

d) Certificate of PhilGEPS registration; and

e) DOT Accreditation Certificate.



VII. INSTRUCTION TO BIDDERS:



Please submit your bid together with the Legal/Technical Requirements stated above on or before 9:00 a.m. of June 6, 2016 to the DOT-BAC Secretariat c/o Mr. Florencio S. Capati, Jr., 4th Floor, DOT Bldg., No. 351 Sen. Gil Puyat Ave., Makati City, Philippines. The date and time of bid opening is June 6, 2016 at 11:45 a.m., 3rd Floor Conference Room, DOT Bldg., No. 351 Sen. Gil Puyat Ave., Makati City. Bid should be duly signed by your authorized representative and should comply with the Terms of Reference (TOR) stated above. Bidders are required to submit seven (7) certified true copies of their bids.
