To participate in the Tender, the Contractor must review and fill out a package of tender documentation (1. Ceiling Specification; 2. Subcontract Agreement - processed and provide a protocol of disagreements); After processing the tender documentation, the Contractor must provide completed package of tender documentation and add the following documents to the package: -Commercial offer - must be signed by an authorized person and certified with a seal, -Commercial offer in Excel format (completed Ceiling Specification with a saved list works and volumes with prices for each item except materials - the Customer's material) - Certificate of arbitrary form, a scan copy of a similar contract and a document confirming its execution (Act of completed works); - Extract from the EDR (full) as of today submission of documents; - Articles of Association in the current version (all pages, including the reverse side, on which there is information); related to the creation of architectural objects with appendices; - Special permits and declarations granted to the Subcontractor for the performance of high-risk works and for the operation of machines, mechanisms, and high-risk equipment; - Certificates of employees about completion of training on OP and TV issues; qualification certificates of employees; - Copy of the passport and code of the manager (signatory) of the Subcontractor; - Power of attorney or other document confirming the authority of the signatory of the Subcontractor (if available); - Balance sheet (Report on financial condition) of the Subcontractor - the last submitted (registered); - Subcontractor's report on financial results (Report on total income) - the last submitted (registered); (income, financial support, assistance, compensation) of insured persons and the amount of the accrued single contribution for mandatory state social insurance to the revenue and tax authorities) Appendix 4 - table 1 Last...
