REQUEST FOR QUOTATION (RFQ)
The Provincial Government of La Union (PGLU), through the PGLU Bids and Awards
Committee (BAC), intends to apply the sum of ₱1, 900, 000.00 to procure the Improvements
of the Provincial Accounting Office/ INFRA-26-00-GF-002 which will be undertaken in
accordance with Section 34 Small Value Procurement of the Implementing Rules and
Regulations of RA 12009.
Interested Suppliers are required to submit the following documents:
a) Mayor-s/Business Permit;
b) PhilGEPS Registration Number (Platinum Membership may be submitted in lieu of the
Mayor-s Business Permit and PhilGEPS Registration Number);
c) PCAB License;
d) Quotation Form, Bill of Quantities and Cash Flow by Quarters; and
e) Notarized Omnibus Sworn Statement.
