Interested suppliers are required to submit their (1) valid/current Mayor-s Permit, (2) Philgeps Registration Number, (3) PCAB License, (4) Income/Business Tax Return, (5) Omnibus Sworn Statement, (6) Performance Security, (7) Certificate from SEC, DTI for sole proprietorship or CDA for cooperatives and (8) Price Quotation. All quotations must be submitted in a sealed envelope showing the control number and name of title of the project being quoted and must be submitted directly to the BAC Secretariat at City Mayor-s Office, City Hall of Balanga. Also, no fax quotation will be honored.
