Documentary Requirements shall include the following:
a) Current Mayor's/Business Permit or in cases of recently expired Mayor-s/Business Permit, it shall be accepted together with Official Receipt for Renewal as proof that the bidder has applied within the period prescribed by the concerned local government unit; and.
b) PhilGEPS Registration Number.
c) Updated Omnibus Sworn Statement (for ABCs above 50k) with Sec. Certificate or SPA (as applicable)
The OSS document must be notarized by counsel/lawyer who is MCLE VIII compliance. If document is not properly notarized, or that failed to show proof of MCLE Compliance (Category VIII) or MCLE exempted, it shall be deemed non-compliant with this requirement.
