hmms, on behalf of the participating hospitals of the south west local health integration network (sw lhin) is issuing this rfp to establish a long term partnership with a common provider for the supply of basic accidental death & dismemberment (ad&d) and optional ad&d insurance. A listing of participating hospitals is provided in appendix 4.

Each of the participating hospitals is responsible for its own ad&d insurance program and for contracting with the successful proponent for common provider. Determination of plan design, policy structures, rating structures, funding arrangements, etc. Are the responsibility of each respective participating hospital.

The participating hospitals are looking to re-establish a long term partnership with a common provider for the provision of ad&d insurance as required for the delivery of each hospital's benefits programs. The expected advantages of the partnership include:

?achieving competitive pricing for each participating hospital individually and for the group of participating hospitals collectively;
?reducing the duplication of effort and resources across the system;
?the ability of leveraging purchasing and contract management to ensure ad&d insurance is procured, delivered and implemented in the most efficient and effective manner;
?the further fostering of partnerships between hospitals and across the lhin.
