REQUEST FOR QUOTATION (RFQ)

For the Procurement of Equipment for Info-Van (Lapel Microphone, Icom Radio, etc.)

For the GO-Public Affairs & Information Assistance Division Office (GO-PAIAD)





1. The GO-Public Affairs & Information Assistance Division (GO-PAIAD) had requested for the Procurement of 2 units Time Attendance Biometric Machine intends to apply the sum One Hundred Forty Four Thousand Five Hundred Pesos (a‚+/-144, 500.00) being the Approved Budget for the Contract (ABC) to payments under the contract for charge to 2017 Capital Outlay (1-07-05-070) to cover eligible payments for which this Request of Quotation is issued.



2. A set of technical specifications are provided in Attachment 1. All items listed under the purchasers specifications must be complied on a pass-fail basis. Failure to meet any of the requirements will result to rejections.



3. Quotation procedures will be conducted in accordance with the provisions of the Implementing Rule and Regulations (IRR) of Republic Act 9184.



4. It is the intent of the Purchaser to evaluate bid/quotation for each item separately, and award will be made to the bid/quotation or combination bid/quotations resulting in the overall lowest cost. Bid/Quotation for one or more or all items will be considered. Contract award will be made to the lowest evaluated quotation(s) meeting purchaser-s technical specifications.



5. Quotation must be delivered at the PGSO, Province of Nueva Vizcaya not later than 9:00 am of January 22, 2019.



6. Your prices must be quoted in the Philippine Peso and must include the unit price and total price, inclusive of all taxes to paid and other incidental cost to delivery site if the contract is awarded.



7. All bids/ quotations must be placed in sealed envelope marked -œProcurement of Equipment for Info-Van (Lapel Microphone, Icom Radio, etc..) for the GO-Public Affairs & Information Assistance Division (GO-PAIAD)- address and The Amount of ABC (a‚+/-144, 500.00) and -œNAME OF COMPANY/ESTABLISHMENT-.





8. Bids/ quotations shall be valid for sixty (60) calendar days from Opening of Bids.





9. The delivery period shall be within fifteen (15) calendar days from receipt of the Purchase Order (PO).



10. DELIVERY SITE: Procurement Office, Province of Nueva Vizcaya.



11. The applicable rate for late deliveries is one tent (1/10) of one (1) percent of the cost of the underperformed portion of every day of delay. The maximum deduction shall be ten percent (10%) of the amount of the contract. Once the cumulative amount of liquidated damages reaches ten percent (10%) of the amount of the contract, the procuring entity shall rescind the contract, without prejudice to other courses of action and remedies open to it.



12. The Province of Nueva Vizcaya reserves the right to accept or reject any quotation, and to annul the bidding process and reject all quotations at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders and to waive any minor defects or infirmities therein and accept such quotation as may be considered advantageous to the government.
