1. All entries must be typewritten/handwritten and all required documents shall be submitted upon submission of quotation.
2. Delivery period is seven to fourteen (7-14) working days upon receipt of Purchase Order / Contract.
3. Shall submit valid License to Operate (LTO) issued by FDA.
4. Price validity shall be for a period of 120 calendar days.
5. Updated BIR Tax Return.
6. Shall submit PhilGEPS registration certificate and Mayor/Business Permit
7. Shall submit FDA CMDN/CMDR for Medical Supplies and Laboratories Supplies.
For Drugs and Medicine (If applicable)
8. For drugs/medicines not previously used by QMMC for the past two years, supplier shall submit samples for evaluation (good for 7 patients- full course) upon determination as the lowest bidder.
9. Shall submit Certificate of Product Registration (CPR) and Certificate of Good Manufacturing Practice (CGMP) for drugs and medicine
10. For I.V antimicrobials such as 2nd and 4th generation Cephalosphorins, Betalactam / Betalactamase inhibitor and Carbapenems shall be compatible with Sterile Water 0.9 NACL, Dextrose 5% or co...
