Offers are invited by department of health in this request for offer hsq101802-05 for the establishment or variation of a standing offer arrangement (soa) pharmaceutical header arrangement (pha) for the provision of general pharmaceuticals - gcsfs (granulocyte colony-stimulating factor); grastims.
Offers are to be submitted on the basis that they conform with the following rfo documentation:
· part one a offer documentation for current soa pharma header arrangement holders
o section a - request for offer details;
o section b - conditions of offer;
o section c - specifications and particulars;
o section d - response forms;
· part two standing offer arrangement (soa) pharmaceutical header arrangement (pha).

the department currently has a state-wide standing offer arrangement (soa) pharmaceutical header arrangement (pha) for the provision of general pharmaceuticals which covers multiple therapeutic groups, and products which are used throughout many areas within each hospital health service.
the existing soa has an estimated spend of $460m (exc. Gst) per annum across all therapeutic groups.
The awarded offeror will have the products added to the schedule d list of products of the standing offer arrangement (soa) pharmaceutical header arrangement (pha).

eighteen (18) months and sixteen (16) days.

mandatory requirements of the goods
the offeror must deliver the goods in compliance with the mandatory requirements, including:
insurances (public & product (goods) liability and workers compensation);
quality assurance (qa) as per as/nzs/iso 9001:2015 and/or iso 13485:2016, and/or tga manufacturing licence and/or wholesale licence;
tga certification for all products (where applicable);
compliance to specification requirements; and
offer and samples lodged in-full, on-time (within 5 business days of being requested)
