Prospective bidders should have experience in undertaking a similar project within the last ______years

(minimum of 2 years) with an amount of at least 50% of the proposed project for bidding, and

have 4 key personnel and equipment (owned, leased or under purchase) available for the prosecution

of the project. The Eligibility Checks/Screening as well as the Preliminary Examination of the Bids shall

use non-discretionary "pass/fail" criteria. Post-qualification of the lowest calculated bid shall be con- ducted.

Security, Pre-Bidding Conference ( s ), Evaluation of Bids, Post-Qualification Award of Contract shall

Checklist of Eligibilty Requirement for Bidders
Eligibility Requirements:
A.) Class "A" Documents

Legal Documents-
i.) Registration certificates from SEC, Department of Trade & Industry (DTI) for sole proprietorship,

or CDA for cooperatives, or any proof of such registration as stated in the Bidding Documents.

ii.) Mayor''s permit issued by the City or Municipality where the principal plaace of businessof the

prospective bidder is located.
Technical Documents-

iii.) Statement of the prospective bidder of all its ongoing and completed government and private

contracts, including contracts awarded but not yet started, if any, whether similar or not similar

in nature and complexity to the contract to bid, within the relevant period as provided in the

Bidding Documents.

iv.) In case of Procurement of infrastructure projects, a valid Philippine Contractors Accreditation

Board (PCAB) license and registration for the type and cost of the contract to be bid.
Financial Documents-

v.) Audited financial statements, showing, among others, the prospective bidder''s total and current

assets and liabilities, stamped "received" by the BIR or its duly accredited and authorized

institutions, for the preceding calendar year which should not be earlier than (2) from the

date of bid submission.

vi.) The prospective bidder''s computation for its Net Financial Contracting Capacity (NFCC) or a

commitment from a Universal or Commercial Bank to extend a credit line in favor of the

prospective bidder if awarded the contract to be bid (CLC).
B.) Class "B" Documents:

C.) Sworn statement by the prospective bidder or its duly authorized-
1.) It is not blacklisted or barred from bidding by the GOP or any of its agencies, offices, corp., or LGUs;

2.) Authentic copy of the original, complete, and all statements and information provided therein are

true and correct;

3.) Authorizing the Head of Procurement Entity or his duly authorized representative/s to verify all

the documents submitted;

4.) The signatory is the duly authorized representative of the bidder.
Bid Security-(Must be valid for 120 CD from date of the opening of bids as determined by the Head of

Procuring Entity)

a.) 2% of ABC- Cash, cashier''s/manager''s check, bank draft/guarantee confirmed by a Universal or

Commercial Bank or Irrevocable letter of credit issued by a Universal or Commercial bank:Provided

however, that it shall be confirmed or authenticated by a Universal or Commercial Bank, if issued

by a foreign bank.

b.) 5% of ABC- Surety Bond callable upon demand issued by a surety or insurance company duly

certified by the Insurance Commission as authorized to issue such security.

c.) Any combination of the foregoing (Proportionate to share of form with respect to total amount

of security).
