The Authority of the Freeport Area of Bataan invites you to quote for our procurement requirement. The details are in the attached file.

3. Copy of the latest Mayor's Permit/Business Permit
4. Income/Business Tax Return (latest submission)
5. Notarized Omnibus Sworn Statement (must be submitted only by the winning supplier prior to award)
6. The bidder shall submit a valid and current Manufacturer-s Authorization Certificate ((MAC) or Authorized Distributor Certificate issued by the pistol grip nozzle manufacturer, authorizing the bidder to supply, distribute, and provide after-sales support for the offered equipment.
7. The bidder shall provide a Certificate of Compliance that the offered pistol grip nozzle complies with National Fire Protection Association (NFPA) 1964.
8. The Winning Bidder shall conduct functional testing of all supplied pistol grip nozzle upon delivery.
