Through this Request for Proposal (RFP) the Agency of Human Services, Department of Vermont Health Access is seeking to strengthen Blueprint-s capacity to advance high-performing primary care practices that deliver coordinated, patient-centered, and population-focused care. This RFP details the proposal submission and evaluation processes, the required experience of the bidder, the scope of work to be performed, and the resulting contract deliverables.
