Endoscopy laboratory equipment 1) The subject of the order is equipment for the endoscopy laboratory 2) The subject of the order is described in detail in Annex No. 1 to the SWZ Product and price form (Description of the subject of the order - OPZ). 3) Detailed conditions i the rules regarding the implementation of the subject of the order are regulated in the Template Agreement, Annex No. 4 to the SWZ (Contractual Terms) 4) The Ordering Party requests that the Contractor indicate in his offer the parts of the order whose performance he intends to entrust to subcontractors and provide Contractor of subcontractor companies - in accordance with Art. 462 section 2 of the Public Procurement Law, if they are already known. 5) If the change or resignation from the subcontractor concerns the entity whose resources the Contractor relied on, in accordance with the principles specified in Art. 118 section 1 of the Public Procurement Law, for the purpose of demonstrating compliance with the conditions for participation in the procedure, the contractor is obliged to demonstrate to the Ordering Party that the proposed other subcontractor or contractor independently meets them to an extent no less than the subcontractor whose resources the Contractor referred to in during the contract award procedure.
