Requirements:

1. Certificate of Product Registration (CPR)

a.) Issuing office is the FDA

b.) CPR that will expire within 3 mos. prior to expiration date should present the official receipt of renewal of application from FDA

2. Certificate of Current Good Manufacturing Practice (CGMP)

a.) for Philippine made product-valid CGMP issued by FDA should be submitted

b. for foreign issued documents, CGMP should be authenticated by phil. embassy

c. validity date should be clearly indicated

3. Latest License to Operate (LTO) as drug manufacturer/drug trader/drug distributor/wholesale from FDA.

a.) LTO that will expire within 3 mos. prior to expiration date must present the official receipt of renewal of application from FDA.



*For CPR & CGMP, photocopies of original, authenticated & marked or stamped "certified true copy"

signed by the issuing office should be accepted. However the original documents should be presented during the evaluation period.

i⤠Delivery: seven (7) to ten (10) days after the receipt of Purchase Order

i⤠VMMC has the option to procure the item in the open market in the event the winning supplier fails to deliver the item/s within the delivery period and shall be charged the difference in price, if there be any.
