1. Object of the order: DRESSING MATERIALS described in detail in Annex No. 2 and 3 to this SWZ - individual packages. 2. Scope of the subject of the order: The goods offered by the Contractor must: a) Have valid documents allowing admission to trade in the territory of the Republic of Poland in accordance with the provisions of the applicable Act of April 7, 2022 on medical devices and the method of classification pursuant to the Regulation of the Minister of Health of November 5, 2010 regarding method of classifying medical devices - if it applies to a given product range, b) Have a permit from the Minister of Health or the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products to place the product on the market medicinal products - products classified as medicinal products within the meaning of the Pharmaceutical Law Act; - if it concerns a given product range; c)Have an EC declaration of conformity and an EC certificate or an EC declaration of conformity with an EC certificate /if applies to/ and notification to the President of the Office of the RPLWMiPB - products classified as medical devices within the meaning of the Medical Devices Act - for all classes of medical devices; d) Have permission from the Minister of Health or the President of the Registration Office Medicinal Products, Medical Devices and Biocidal Products on the trade of a biocidal product.
