The subject of the order is: Successive deliveries of tubing for the Stryker laparoscopic tower. The goods offered by the Contractor must: a) Have valid documents allowing for admission to trading in the territory of the Republic of Poland in accordance with the regulations the applicable Act of April 7, 2022 on medical devices (Journal of Laws of 2022, item 974), its transitional and implementing provisions and the EU Regulation 2017/745 on medical devices - MDR (if the law imposes an obligation to have such documents) - if it concerns a given assortment, b) meets the requirements specified by the Ordering Party in the SWZ, in particular the conditions specified in the description of the subject of the order. A detailed description of the subject of the order, the description required by the Ordering Party in the scope of implementation and acceptance specify: Assortment and price form, Appendix No. 2 to the SWZ, draft provisions of the contract, Appendix No. 3 to the SWZ.
